To make a referral to our service, please download our Referral form. Download the form below (in Word 2007 format) and complete it either electronically or by hand. Once completed, it can be emailed to [email protected] or faxed to 0161 839 3335. You should receive a response within 24 hours of your referral with an appointment for our qualified social worker to conduct an assessment of the user’s needs. All users to our services will undergo an assessment, which is in line with the Common Assessment Framework (CAF). We have found common ground across the BME communities that we work with, and we have modified the CAF to accommodate this.
